So a lot of the people have confussion about the learning outcome forms.So I will try my best to make it easy for you guys.You need following things first before you start filling the forms,leave it for the end.
1.Syllabus
2.Professional References
3.CPD
4. Syllabus of any other exams or Degrees you have done, It’s not mandatory but It makes your application stronger(I attached ORE1 and MFDS 1 syllabus as I have passed these)
Make sure you have got your syllabus translated if its not in English.Now coming to the filling,
NOTE: Learning outcome forms of hygienist and therapist are the same apart from some sections which are additional for the therapist as their scope of practice is more than hygienist.

Now there are 3 columns in learning outcome form.
Let me give you an example of how to fill.
If all of you see Page 6 of form the 

 Column 1 :DTher -1.1.1 which states;

Describe the principles of an evidence-based approach to learning,clinical. and professional practice and decision making:

Now in front, there are 2 column 

Column 2:Where in English translation of your syllabus can this evidence be found:👉🏻

Here you have to mention the topic name and page no. where you have covered this point no matter in which year of your undergraduate you have covered it, can be 1st year or final year of your bachelors or you can write about the topic name and page no.of this point covered in your either additional qualification like ORE, MFDS,any masters.

For example, I say:

BDS syllabus: Community and preventive dentistry page 25, Physiology page 7 and 8

Ore 1 syllabus: Oral Pathology and microbiology page 10

Mfds syllabus:Oral Pathology and microbiology page 3

Like this mention about the topic name which you think has covered the above point and mention page no on your syllabus so they match it that you have covered.

Column 3:Where is this covered in your additional training or work experience:👉🏻

Mention about the Cpds you have done which is related to this along with the date you have done this it can be 1 or more,mention about those which you think are related to this point  and about professional references of any of your referee which has mentioned about this in his reference along with date.

For example:

CPD titled Evidence based decision making:Introduction and formulating good clinical questions dated:12/02/2020

Professional Reference:Dr abcd dated 12/02/2020

Similarly fill all the learning outcome form,Now the space to type in columns is very limited so you can make additional covering letter where you can continue writing,you just make these column mention the point no. and write additional things there.

I will upload a sample of how to do it,for more clarification.Hope it helps.

Hi Nadiye.

For 2nd column of 1.7.10 mention cpd staff involvement ,do it from isopharm and mention about professional reference like I mentioned these 2.My professor has mentioned in reference form about team work and stuff so you can mention about these two in work experience column.

Hi Tashfeen , could you please help ; how to fill DH 1.1.11 and DH 1.2.3 For dh1.2.3 I have smoking cessation and alcohol guidelines from isopharm but I think this are not enough.

Hi Nadiye,

For 1.2.3 I have done cpds.

1.Periodontal disease pathology.

2.Oral cancer sign and symptoms.

You can mention about smoking cessation one in this block.

For 1.1.11 I have done.

1.Tobacco101.Aguide to working with nicotine addicted patients.

2.An introduction to substance usedisorder for dental care professionals.

3.Smoking cessation advices you can also mention here and alcohol one as well.

You can mention ame cpd with different points if you think they are relevant.

I have done all this CPDs , although I did nearly all cpd s from isopharm and dentalcare sometimes it can be so confusing to fill the form..

you have to mention about the awarding body which issued you the degree not the name of your college but the body which is affiliated with ur university mostly its written on ur degree like the name.

Cpd:Treatment coordinator(part 4)provision in practice

1.7.9:cpd delegation and referrals(isopharm)

Cpd:General data protection regulation

Cpd:Aspect of consent and best interest

Cpd:Legal and ethical(course2)philosophical and sociological principles of dental ethics.

1.8.4:Cpd:Equality and diversity in dental practice

Cpd:Maintaining and protecting patient information.

i found this CPD on dentalcare.com giving CE hours & i couldnt find it on the UK Website dentalcare.co.uk? will it still count as CPD hours?
