Hygienist/Therapist Reference Letter

Dear doctors I was wondering if anyone can give an advice regarding a reference letter for hygienist/ therapist.
Please refer to the enclosed screenshot as a reference guideline set by GDC.
I have question regarding the guideline:
1) level of supervision – is it sufficient to mention in a single sentence that I am independent and require no supervision.
2) I am not sure how to write for level of skill – shall I also just mention in a single sentence saying i am ‘ advanced’ for the procedure I mentioned?
Please have a look at my reference letter that I have drafted:
I am writing this letter of reference on behalf of Dr xxx who is an associate dentists in yyy clinic. I was recently informed by xxx of her interest to register as a dental hygienist in the UK and would like to offer my strong recommendation for her.
Dr xxx has been working in the clinic since November 2018. She is able to work independently without any supervision. She has the ability to work well with any patient and is well-liked by all members of staff.
The procedure that she carried out regularly includes
i) intra and extra oral examination and diagnosis
i) oral prophylaxis and scaling
ii) restorative treatment ( cavity fillings and sealant applications)
iii) endodontic treament
iv) oral surgery including simple, complicated extraction and impacted third molars removal
v) paediatric and geriatric dentistry
vi) denture construction
vii) administration of local anesthesia, medication and pain killers
viii) conduct and interpret diagnostic procedure ( Orthopantomogram and periapical radiography)ix) advanced prosthodontic procedure (crown and bridge)
x) emergency dental care ( dental abscess and dental trauma management)
On top of that, she is also knowledgable and experienced in:
1) follow proper decontamination and sterilisation guidance
2) obtain and record good patient history
3) know self limits and refer patients appropriately to other members of dental team or other health professionals4) explain the impact of medical and psychological conditions in the patient
5) recognise the need to monitor and review treatment outcomes
6) recognise and take responsibility for the quality of care provided to the patent
7) take responsibility for ensuring compliance with current best practise guidelines8) provide patients with comprehensive and accurate education and instruction in a manner which encourages self-care and motivation
9) assess the results of treatment and provide appropriate aftercare and on going preventive advice10) explain evidence-based prevention and apply appropriately
11) Good leadership skill and able to work as a team12) Recognise and demonstrate own professional responsibility in the development of self and the rest of the team
13) act without discrimination and treat all patients with equality, respect and dignity
14) Prioritise and protect patients’ interests, dignity and choice
15) Act with integrity and respect patients’ choices and keep them informedShe is a person to work with and a dedicated dentist who works tirelessly providing the best possible care for her patient.
